Patient Portal

The Patient Portal is designed to assist with communication between the clinic and you as a guardian to provide the best possible care for your child.

Once you choose to use the portal, a link will be sent to the email address you have given us.  If you do not have one, please ask our front desk staff to send you an email for portal access.  Please log onto the portal with the code provided to you in the email and create your personal password.  You will then have access to your child’s medical records through our portal at www.ConcordChildrensClinic.com.

Once your child turns 18, parent access is suspended.  The “Adult” child will need to physically come to Concord Children’s Clinic and register to “Self-Access” or authorize “Continued Access” for their guardian.

We ask that you request all prescription refills through the portal.  It is quicker and less time-consuming than a phone call.

The portal will give you access to the following items:

  • Messages
    • Inbox
    • Sent Messages
  • Refill Requests (If a visit is required for a refill, an appt. will need to be made first)
  • Encounter Lists
  • Medical Records
  • Lab Results
  • Patient Documents
    • Uploaded by me (uploaded by patient/guardian to clinic)
    • Shared Form Folder (documents from clinic to guardian)

If you have signed up to use the Patient Portal, THANK YOU!  We hope it helps you access the child’s information as well as improves communication with our clinic.

If you have not signed up to use the Patient Portal, we hope that you will CONSIDER SIGNING UP SOON!  We find those patients/guardians who use the portal can access information much easier than having to call the clinic.
